Recent trends in kidney transplantation have included a dramatic improvement in short-term outcomes, reflecting better understanding of immune mechanisms leading to allograft rejection and the development of potent new regimes of immunosuppressive drugs that effectively prevent and treat rejection. Concurrent surgical innovations have included new techniques for harvesting multiple organs, laparoscopic organ removal, and novel strategies for surgical management of marginal donors and recipients who would have been rejected in an earlier era. Attention has turned increasingly to strategies that optimize long-term allograft survival by minimizing the major factors contributing to posttransplant mortality.
