Before the season's start, Terry Cooper replaced Lou Macari as manager on 9 August 1991, after he joined Stoke City. Birmingham gained promotion by finishing second in the league. In the cup competitions, they suffered an early exit in the FA Cup, losing 3–0 to Torquay United, and in the League Cup, they reached the third round, having previously beaten Exeter City and Luton Town, before losing to Crystal Palace after two replays. They failed to progress past the preliminary round of the Football League Trophy.
